Proximal humeral comminuted fracture, especially the one has apparent displacement, is difficult to perform closed reduction surgery as the traction caused by the fracture which attached with rotator cuff muscles in greater and lesser tubercles, thus conservative therapy often cause deformity and cause severe influence on joint activities.
For the confirmed cases, the department usually uses surgical reduction internal fixation LPHP technique; for old patients combined with severe osteoporosis, manual closed reduction with external cage fixation is recommended, which gets good curative effect. For patients with displacement is more than 1cm in length or 45° in curvature, surgical reduction is recommended. For patients with “three parts fracture” or “four parts fracture”, the following plans are recommended, such as closed reduction with percutaneous Kirschner wire fixation, needle nail fixation, open reduction with plate fixation.
